Outreach Support
We have a small team of outreach workers, providing services Monday to Friday between the hours of 7am – 5pm (with some flexibility).
Our Outreach services support people who live in their own home and need support with some day-to-day tasks.
At present we work with people for between two and six weeks and provide individual care packages which include support with:-
- goal setting
- strategies to keep safe indoors and outdoors
- support to attend local activity's and going out in the community.
- maintaining relationships with family and friends
- participating in the community
- independent living skills
- development of social skills
- maintaining / seeking employment or voluntary opportunities.
- any other support they may require in the house be personal care or meal preparations
